Until the services of the Louisiana Vital Records Office are restored, the Arkansas Vital Records Section of the Division of Health will assist in obtaining certified Birth Cards for those needing a Louisiana birth certificate. There will be no fees for the service.
Louisiana procedures specify that application must be made to qualified Health Division personnel in person. Arkansas Vital Records has made arrangements to accept applications in its Little Rock, El Dorado, Fort Smith, and Fayetteville offices. Persons unable to reach one of these offices may call (501) 661-2275 for assistance.
Evacuees:
- in central Arkansas may file an application with Vital Records personnel at the Division of Health main office at 4814 W. Markham St., Little Rock.
- in southern Arkansas may file an application with the Union County Health Unit Registrar at 301 American Road, El Dorado.
- in western Arkansas may file an application with the Sebastian County Health Unit Registrar at 3112 S. 70th Street, Fort Smith.
- in northwest Arkansas may file an application with the Washington County Health Unit Registrar at 3270 Wimberly Drive, Fayetteville.
Applications will be electronically transmitted to the Louisiana office. Arkansas Vital Records should receive the requested Birth Cards within a week after submitting each request. Arkansas Vital Records will distribute the cards to evacuees.
