      Please review this form before you come to the mass flu clinic. You can fill out a form at the clinic. You can also print out the form, fill-in your information and bring it to the clinic. Please note: the form must be printed on one sheet of legal-size paper front and back. Clinics will not accept a form printed on two separate sheets of paper.

FluSeasonal flu is a disease that causes mild to severe illness. Each year in the United States, there are 25-50 million infections, over 200,000 hospitalizations and roughly 36,000 deaths due to flu. Of those hospitalized, 20,000 are children younger than five years old. Over 90 percent of deaths and about 60 percent of hospitalizations occur in people older than 65.

What are the symptoms of seasonal flu?

Symptoms of seasonal flu include: fever greater than 100 degrees, body aches, coughing, sore throat, chills, headache and body aches, fatigue, respiratory congestion, and in some cases, diarrhea and vomiting. Anyone experiencing these symptoms should contact their physician or other health care provider.
