How do I get a medical exemption for my child to not get an immunization?

A physician statement outlining medical reasons for an exemption of a specific immunization is to be submitted to the Medical Director of the Division of Communicable Disease/Immunization,

The Medical Director will decide whether to grant the medical exemption based on the CDC guidelines.

How do I get a religious exemption for my child to not get any immunizations?

Contact the Division of Communicable Disease/Immunization at (501) 661-2169 to request the appropriate form. Religious exemptions can be granted for children "on the grounds that such immunization conflicts with the religious tenets and practices of a recognized church or religious denomination of which said parent or guardian is an adherent or member" in accordance with Arkansas Statute.
