As a licensed daycare facility in Arkansas, how can I access and view the Immunization Network for Children (INC) registry?

Download and print the following form, Application to Access (WORD), and return it to the address listed on the form via e-mail, fax or mail. After the Immunization Section receives your application, they will contact you with a user name and password. Once you receive your personal user name and password, the following documents will direct you to accessing the registry and offer a tutorial on locating an immunization record.

Patients should contact the clinic or provider who gave the immunization.

How do I get a medical exemption for my child to not get an immunization?

A physician statement outlining medical reasons for an exemption of a specific immunization is to be submitted to the Medical Director of the Communicable Disease/Immunization Section with the exemption form.

How do I get a religious or philosophical exemption for my child to not get any immunizations?

Religious exemptions can be granted for children "on the grounds that such immunization conflicts with the religious tenets and practices of a recognized church or religious denomination of which said parent or guardian is an adherent or member" in accordance with Arkansas Statute.
