As a licensed daycare facility in Arkansas, how can I access and view the Immunization Network for Children (INC) registry? |
Download and print the following form, Application to Access (WORD), and return it to the address listed on the form via e-mail, fax or mail. After the Immunization Section receives your application, they will contact you with a user name and password. Once you receive your personal user name and password, the following documents will direct you to accessing the registry and offer a tutorial on locating an immunization record.

| Patients
should contact the clinic or provider who gave the immunization.

How do I get a medical
exemption for my child to not get an immunization? |
A physician statement outlining medical reasons for an exemption of a specific immunization is to be submitted to the Medical Director of the Communicable Disease/Immunization Section with the exemption form.

PLEASE NOTE: The form requires a signature and seal of a Notary Public.

The Medical Director will decide whether to grant the medical exemption based on the CDC guidelines.

How do I get a religious or philosophical
exemption for my child to not get any immunizations?
|
Religious exemptions can be granted for children "on the grounds that such immunization conflicts with the religious tenets and practices of a recognized church or religious denomination of which said parent or guardian is an adherent or member" in accordance with Arkansas Statute.
