The Division of Vital Records iis open Monday through Friday, 8:00 a.m. until 4:30 p.m. The building is closed on state holidays and weekends.

What is the cost of a birth certificate?

The cost of a birth record is $12.00 for the first copy and $10.00 for each additional copy ordered of the same record at the same time. If no record is found or no copy made, state law requires that we keep $12.00 for a searching fee. The customer may request a refund of any amount paid over the required $12.00 search fee.

What is the cost of a death certificate?

The cost of a death certificate is $10.00 for the first copy and $8.00 for each additional copy issued at the same time for the same certificate. If no record is found or no copy is made, state law requires that we keep the $10.00 for a searching fee. The customer may request a refund of any amount paid over the required $10.00 search fee.

What is the cost to search the files for a birth or death certificate?

The search fee for a birth record is $12.00 and the search fee for a death record is $10.00.

Who may apply for a record without the need of written authorization or documentation indicating need for the copy?

The individual named on the record, specific family members and designated representatives.

All other applicants must submit written authorization from one of the eligible family members, or provide documentation-proving need of the copy.

I am working on my family tree (genealogical research), can I get a copy of records on family members?

Family members who are conducting genealogical research and genealogists
representing relevant family members may obtain copies of vital records. Birth records occurring over 100 years ago are open to the public and deaths which occurred more than 50 years ago are also open to the public. The Arkansas History Commission has an index of deaths for 1914 - 1949.

You may expedite your request with a credit card (Visa, Master Card, Discover or American Express) by calling toll free 866 209-9482 or on the Internet at www.vitalchek.com. Your request will be processed the next business day. The certificate or a response to your request will be returned by regular mail in seven to ten business days or it maybe returned by Federal Express for the additional shipping fee. Call and verify fees as they may change without notice. Vital Records cannot fax copies of records.

How long will it take to receive a birth certificate requested through regular mail?

To receive a certificate through the regular mail it will take four to six weeks.

How do you correct information on a certificate?

You may mail or bring a copy of your birth certificate, indicating the information that is in error, and furnish us with the correct information. We will mail you instructions on how to have the information corrected.

We just adopted a child born in Arkansas.  What do we need to send to your office?

Send to us a certified report of adoption or a certified final decree for adoption from the county clerk's office where the adoption was finalized. The fee is $27.00 for preparing the new certificate and one certified copy of the new certificate.

How do I add the father's name to the birth certificate when the parents did not marry?

You will need an Acknowledgement of Paternity form, which maybe obtained from our office or the Office of Child Support Enforcement. Both the mother and father must sign this form and have it notarized. You can only use this form if the mother was not married at the time of birth and there is no father listed on the birth certificate.

I married the natural father of my child. How do I add his name to the birth record?

We will need a certified marriage license and the affidavit signed and notarized. We can only do this process if the natural parents married after the child was born. The mother cannot be married to another man before the child was born.

How do I remove the father's name from a birth certificate and change the child's name?

You will need a court order directing this office to remove the father's name from the certificate. The same court order can be used to have the child's name changed. We will remove or add only the information the court order directs us to remove or add.
